These Dialysis facilities range in size from 4,500 SF to 8,000 SF.  Three are new facilities for Davita.  They are located in Freeport, DeKalb, and Rockford.  Dixon, Monroe, and Churchview are addition and renovation projects.  Additional locations are currently in the design phase.
